ENHANCED CARE MANAGEMENT (ECM)
We understand that certain health conditions can be complex, confusing, and hard to manage in your life. ECM offers supportive services to address your whole health—to care for your body and mind. You may qualify for ECM if you are enrolled in Kern Family Health Care, meet certain criteria shown under “WHO IS ELIGIBLE?” and need more help with managing your health. Participation in this program is at no cost to you and will not impact any of your current benefits. ECM provides care coordination, healthcare navigation, and linkage to community resources to meet social needs such as housing, food, and other supports that promote improved health.
WHO IS ELIGIBLE?
ECM is for members who have:
- Homelessness with complex health and/or behavioral health needs
- A serious mental illness or substance use disorder with complex social needs
- Complex needs and are transitioning from incarceration in Kern County
HOW CAN OUR CARE TEAM HELP YOU?
ECM will get you a care team with a care manager who coordinates no-cost services, such as primary care, behavioral health, specialty health, oral health, and social services.
ECM INCLUDES:
- Finding Doctors and getting appointments for physical, mental, and substance use health needs.
- Keeping all your Providers fully informed.
- Setting up transportation to your Doctor visits.
- Getting follow-up services after you leave the hospital.
- Managing all your medicines
Getting help connecting to local resources, such as food or other social services. - Having access to the activity center for groups and appointments with care managers.
