What Is ECM?
ECM (Enhanced Care Management) is a Medi-Cal program that provides personalized, coordinated support for individuals facing complex health challenges. The program focuses on whole-person care—addressing both medical and social needs like housing, food access, and ongoing care coordination.
Who is ECM for?
This program is designed for people who:
- Are experiencing homelessness and have serious health or mental health conditions
- Are living with a serious mental illness or substance use issue and need help connecting to services
- Are leaving jail or prison and need support getting back on their feet
These are often the people who fall through the cracks and we’re here to help them stay connected, safe, and supported.
Who qualifies for ECM?
ECM is available to Medi-Cal members who meet specific criteria, including individuals experiencing homelessness with complex health needs, those with serious mental illness or substance use disorders, or individuals transitioning from incarceration with complex care needs.
Does ECM cost anything?
No. ECM services are completely free and do not affect your existing Medi-Cal benefits. You can join or leave the program at any time.
What kind of support does ECM provide?
ECM offers care coordination, appointment scheduling, transportation setup, medication management, hospital follow-up care, and connections to community resources such as housing and food assistance.

Coordinated Medical & Mental Health Care
Get help finding doctors and scheduling appointments for your physical, mental, and substance use health needs. You’ll have direct access to psychiatrists (no authorization needed) and a dedicated care team including a primary care provider, care manager, and behavioral health support.
Transportation & Access to Services
We arrange rides to your medical appointments—so you never miss the care you need. You’ll also have access to dental and vision services, addiction support, and our day activity center for ongoing wellness and community engagement.
Whole-Person Care Management
Our team keeps all your providers informed and works with you to manage medications, follow-up care after hospital visits, and 24/7 nurse support. Health education and community linkages ensure your needs are fully supported.
Connection to Local Resources
We help you get connected to food programs, housing support, and other essential social services. Your care manager ensures every aspect of your health—body and mind—is addressed through personalized, compassionate coordination.
