Health challenges can be overwhelming—but you don’t have to face them alone. Through Enhanced Care Management (ECM), we offer no-cost, personalized support for your physical, mental, and social well-being.
Who Qualifies for ECM?
You may be eligible if you are a Kern Family Health Care member and meet one or more of these criteria:

Experiencing homelessness and facing complex medical or behavioral health needs

Living with a serious mental illness or substance use disorder and in need of social support

Transitioning from incarceration and facing complex health or social challenges
What ECM Can Do for You?
Your care team, led by a dedicated care manager, helps connect you to essential services—all at no cost:

Medical Care: Help finding doctors and scheduling appointments for physical, mental, and substance use care

Ongoing Support: Coordinating care between your providers to ensure nothing is missed

Transportation: Setting up rides to medical visits

Post-Hospital Care: Ensuring you get the follow-up services you need after discharge

From primary care to behavioral health, our goal is to help you manage your conditions and improve your overall well-being with dignity and ease.
Whether it’s finding doctors, managing medications, arranging transportation, or connecting you to food, housing, and wellness services—your dedicated care team is here to guide you every step of the way. All services are offered at no cost, making it easier to access the support you need.
Coordinated Medical & Mental Health Care
Get help finding doctors and scheduling appointments for your physical, mental, and substance use health needs. You’ll have direct access to psychiatrists (no authorization needed) and a dedicated care team including a primary care provider, care manager, and behavioral health support.
Transportation & Access to Services
We arrange rides to your medical appointments—so you never miss the care you need. You’ll also have access to dental and vision services, addiction support, and our day activity center for ongoing wellness and community engagement.
Whole-Person Care Management
Our team keeps all your providers informed and works with you to manage medications, follow-up care after hospital visits, and 24/7 nurse support. Health education and community linkages ensure your needs are fully supported.
Connection to Local Resources
We help you get connected to food programs, housing support, and other essential social services. Your care manager ensures every aspect of your health—body and mind—is addressed through personalized, compassionate coordination.
